DD Medical Matters: What is Lupus?

first_imgThis week  Dr Ciarán  Roarty of Scally McDaid Roarty Medical Practice takes a look at an interesting condition called Lupus  (Systemic Lupus Erythematosus).Lupus or Systemic Lupus Erythematosus (SLE) to give the full title, is a chronic disease that causes inflammation in different parts of the body and can cause a variety of symptoms depending on what part is affected. These range from tiredness (due to anaemia) depression, anxiety,  right through to joint pains, headaches, chest pain and even mouth ulcers. It tends to affect women more than men and about 1 in 1000 people are affected. The typical patient is female in her 20s, 30s or 40s.  It is caused by the body’s own immune system attacking the body’s own cells. It is not fully understood what causes it and while there seems to be some hereditary factor, there are other factors involved also. Sunlight, infections and even certain medicines may have a role. How does it affect the patient?Joint and muscle pain are very common in SLE, particularly in the small joints of the hands and feet. The pain can “hop” from one joint to another and stiffness is more common in the morning. Rashes may develop, particularly a very specific type of “butterfly” pattern rash on the face, but rashes elsewhere are common also. Mouth ulcers are common in people with SLE as well as hair thinning. Anaemia is common, though usually mild and lymph nodes may swell. Pleurisy (inflammation of the lining of the lungs and inner chest wall) and chest pain may occur as well as problems with the kidneys. Depression and anxiety are not uncommon as well as headaches. Tiredness is also a common feature. Symptoms tend to flare up from time to time and then settle again. How will I know if I have it? If your doctor suspects Lupus they will send certain blood tests.  Once diagnosed, regular monitoring of blood tests are required. Other scans and x-rays may be required depending on the part of the body affected. Can it be treated?We cannon cure Lupus but we can usually help ease the symptoms. Specialists will oversee treatment but usually medications such as anti-inflammatory drugs and even steroids are used. More severe cases may require medicines to settle the immune system. Will I get better? The outlook for patients with Lupus is very good and active normal life is usually possible. Treatments are good and while joint pain and skin symptoms are annoying they can usually be managed well with medicines. Serious complications tend to occur within the first 10 years so after that, they are unlikely to occur. Severe disease where the kidneys or brain are affected can be life-threatening but modern immunosuppressive treatments have greatly improved outcomes even in these rarer cases. Lupus can cause an increased risk of miscarriage but in the case of mild or well-controlled Lupus, pregnancy tends to uneventful.  Kidney involvement may cause blood pressure problems, however. The above information is intended as advice only and should you have any concerns please contact your own Doctor. Dr Ciarán Roarty MB, BCh BAO MICGP DRCOG Grad. New drive for Gauteng tourism

(Image: Janine Erasmus) Building on the momentum of the 2010 Fifa World Cup, the Gauteng Tourism Authority launched a new campaign in late September to promote domestic tourism in the province.The event coincided with the country’s Tourism Month, which this year focused on a variety of local destinations – one of which is Limpopo province, with its wealth of cultural and natural attractions, including the world-famous Kruger National Park.“The platform is there,” said Gauteng MEC for Economic Development Firoz Cachalia at the campaign’s launch. “Tourism shouldn’t be completely dependent on international tourism, so we want to encourage our residents to enjoy their amazing place.”Gauteng premier Nomvula Mokonyane agreed, saying that many Gautengers have a limited knowledge of their own province. “We may not have access to a beach, but everything else is here in Gauteng. However, thus far we haven’t been able to sell this jewel to the locals.”Mokonyane added that tourism should also teach people about their heritage. “It’s all here in Gauteng – we have lions, Mrs Ples, Constitution Hill, Maropeng, Soweto. But to fully take advantage of these treasures we must integrate public transport.”The provincial government is currently revamping the public transport system, with a new bus rapid transit system and high speed rail service under construction – but for carless people to be able to reach these scenic spots, more must be done.Another important aspect of a heightened tourism industry is job creation, while entrepreneurs such as B&B owners will also benefit. The provincial government has plans to construct an extensive database of tourism operators and B&B owners, and link these to established facilities, so that visitors can put together a package to suit their pocket.“People can grow their businesses by targeting local tourists,” said Cachalia. “You don’t have to depend on international visitors. With better domestic tourism, people will develop better products for the local market, and by extension, the international market.”A more lively domestic tourism sector will lead to sustainable international tourism, said Mokonyane. “A country that doesn’t promote domestic tourism can’t expect other tourists to come, because the locals won’t know where to take them.”The Gauteng Tourism Authority has a training programme for tourist guides, said chairperson Khehla Mthembu, and also registers them on the website as qualified and approved by the organisation.The new campaign, called The Amazing Place, is not only aimed at Gauteng residents – it will run nationally as well.Domestic investmentAccording to the Department of Tourism, some R70-million (US$10-million) was spent since 2004 to promote South Africa’s diversity of destinations. This sum encompasses a range of media genres, including outdoor, TV, print and radio advertising, as well as specific events.According to figures available from South African Tourism, South Africans made some 30.3-millon domestic trips during 2009, whether for holiday, business, medical, religious, or other purposes.Overall, most travellers came from Gauteng and KwaZulu-Natal provinces, with 29% and 25% of the total number of trips respectively. KwaZulu-Natal and the Eastern Cape were the most popular destinations. Of the holiday travellers, most came from Gauteng and were headed for the Western Cape.The local travellers spent a total of R22.4-billion ($3.2-billion) on their trips, with an average of 4.2 nights per trip.The figures also show that of all the provinces, Gauteng residents are the ones who most prefer to travel to other provinces – just 32% of them travel intra-provincially. This means that the Amazing Place campaign has come at the right time.Race around the provinceAn important element of the Amazing Place campaign is an exciting TV show, based on the popular Amazing Race programme which keeps millions of viewers around the world glued to their screens.The Gauteng Tourism Authority was supported in this initiative by local radio station Kaya FM and Chevrolet, who supplied the cars.“After the World Cup we are stepping up the action with this unique race that highlights Gauteng’s tourism routes both to outsiders and residents within the province,” said Mokonyane.Filming took place at the end of September and saw eight plucky contestants in four teams making a 48-hour dash around the province, chasing clues and hoping to be the first across the finish line. Along the way they experienced some unusual sights and sounds, and were treated to the best the province has to offer, as well as the diversity of its culture.The series, which will comprise at least four episodes, is scheduled to air at the beginning of November on DStv bouquet channels and the SABC. It will be presented by Kaya FM’s Bob Mabena.Teams were made up of local celebrities. They were Department of Economic Development communications manager Uyanda Mbuli with singer, songwriter and record label owner Phinda Matlala; recording artist Selaelo Selota with model Cwaita Mtose; house music vocalist Lelethu Nkulu with Kaya FM afternoon show host George Monetsi; and Jam Alley host Thato Mahlatsi with up-and-coming musician Malik.The race started in the now-defunct women’s prison in the Old Fort at Constitution Hill in Johannesburg. First a fort and then a notorious prison, the site has undergone a remarkable reversal of purpose and is now the seat of the highest court in the land. The women’s prison was added 15 years after the original fort was built in 1892.On the first day the contestants visited the 94ha Rondebult Bird Sanctuary in Ekurhuleni east of Johannesburg, the Vaalnest Boutique Hotel on the banks of the Vaal Dam in the tiny town of Vaal Marina in south Gauteng, and the ever-popular Maropeng in the Cradle of Humankind, which was also the pit stop for the day’s proceedings.On day two they set off for the nearby Magaliesberg Mountains and a lofty appointment with Bill Harrop, who runs a popular hot-air balloon safari company, before heading north for Freedom Park in Pretoria and a glimpse of the country’s heritage. The park is a monument to democracy and features a Wall of Remembrance, inscribed with the names of those who fell during the struggle for freedom.The race ended at the magnificent Nanhua Buddhist temple, the largest Buddhist seminary and temple in Africa. At ReadWriteMix, he told the crowd that the future of Foursquare isn’t the check-in. Instead, it’s two completely separate activities that now live in two different Foursquare-made apps.Soon Foursquare will no longer be an app dominated by a check-in button. Indeed, it won’t even have one. Instead, the company is turning its flagship application into a Yelp-like location recommendation service that learns your behavior and sends you notifications on your phone it thinks you might find helpful.See Also: Why Foursquare Is Breaking Up With The Check-InFor instance, if I regularly eat at burrito places in San Francisco, and I have Foursquare location service turned on, it will monitor those locations and learn where I go. For instance, urban planners look at check-in data as they build out cities and suburban areas, he said.The Potential Of Empowering BusinessThis radical change to Foursquare’s product comes at a time when the business finally seems to be hitting its stride.“In the first two quarters of this year, we’ll make more money than we did all of last year,” he said.Each check-in on Foursquare—soon, on Swarm—contributes to what Crowley calls “venue polygons.” These location shapes define a specific business or place, and can be used to create markers for geofences that will be able to tell when you’ve entered a specific location.At this week’s Apple Worldwide Developers Conference, many people expected Apple to talk about iBeacon, its Bluetooth-powered product that can alert shoppers to deals or recommendations, as well as allow people to make payments from their smartphones, when they enter a place of business, or even a particular section of a store. Apple didn’t announce any information about iBeacon this week, but at ReadWriteMix, Crowley talked about Foursquare’s potential to provide businesses with similar options.Where iBeacon depends on Bluetooth transmitters which can transmit data to a smartphone, Crowley says Foursquare has the capabilities to map places purely through its software and data, without draining people’s smartphone batteries. If Foursquare can offer similar capabilities by leveraging smartphone location data and partnering with different businesses, people might soon be able to step into a store, read a suggestion, and make a purchase, all from the palm of their hand—and the business wouldn’t have to do much at all. Of course, Crowley’s vision for a Foursquare that tracks your ambient location data and knows where you are at all times brings up serious privacy concerns. As more people become wary of sharing private data with both friends and social networks, the question becomes whether people want or need a service that is all-knowing, and all-tracking. “Our intent is to make these magical pieces of software that teach you about the world,” Crowley said. On both Foursquare and Swarm, you can turn the ambient location sharing off—but, of course, that takes away from some of the benefits you get by downloading them in the first place. Like any other social network, if you want to use Foursquare or Swarm for convenience, you’ll have to trust your data to the company. For some, it’s worth it. Franklin, File)DALLAS (AP) — The declaration by the powerful owner of the Dallas Cowboys that he would bench anyone who shows disrespect to the American flag drew a sharp response from the NFL players’ union Monday and raised the possibility of another call to action by athletes who have kneeled during the national anthem.Executive Director DeMaurice Smith of the NFL Players Association said the most provocative comments yet by Jerry Jones on the anthem controversy contradicted assurances last week from Commissioner Roger Goodell and New York Giants President John Mara that players could express themselves without retribution.“I look forward to the day when everyone in management can unite and truly embrace and articulate what the flag stands for, liberty and justice for all, instead of some of them just talking about standing,” Smith said. “We look forward to continuing our talks with them on this very issue.”Jones’ comments on Sunday came after he was asked about Vice President Mike Pence’s decision to leave an Indianapolis home game in protest of about a dozen San Francisco players who kneeled during the anthem. President Donald Trump tweeted after Pence’s walkout that he had told his vice president to leave if any players kneeled during the anthem.Following a 35-31 loss to Green Bay, the 74-year-old Jones said the NFL cannot leave the impression that it tolerates players disrespecting the flag and said any Cowboys doing so will not play.“If there’s anything that is disrespectful to the flag, then we will not play,” said Jones, who was inducted into the Pro Football Hall of Fame in August. “OK? Understand? If we are disrespecting the flag, then we won’t play. Period.”The Cowboys and Jones kneeled arm-in-arm before the anthem when they played at Arizona two weeks ago, a few days after Trump said at a rally in Alabama that NFL owners should fire any players who disrespect the flag. All of them stood during the anthem, with arms still locked.Most Dallas players have stood on the sideline, many with hands over their hearts, during the anthem since former 49ers quarterback Colin Kaepernick started kneeling last season in protest of police treatment of African-Americans.Several NFL teams have struggled with how to handle anthem protests. After Trump’s criticism, the Pittsburgh Steelers agreed to stay off the field before the anthem. But Army veteran Alejandro Villanueva, an offensive lineman, stood at the edge of a tunnel with his teammates in darkness behind him during the anthem two weeks ago.Villanueva said he was not making a political statement in defiance of his teammates, calling it a misunderstanding that was “very embarrassing on my end.”Miami coach Adam Gase recently set a new team policy requiring players either to stand or wait in the tunnel. Three chose to stay off the field, Michael Thomas, Kenny Stills and Julius Thomas. All three have kneeled in the past.“We were just trying to keep the team focused and not be a distraction,” Michael Thomas said. “It was a team decision. … The league heard us. They’ve heard the cry of the players.”Mara has told Giants players he wants them to stand during the anthem but supports their right to do otherwise. Jones had already made it clear that he felt strongly about standing for the anthem before his latest comments, and he isn’t the only owner who feels that way.Jones even suggested that standing for the anthem was more important to him that team unity.“The main thing I want to do is make it real clear: There is no room here if it comes between looking non-supportive of our players and of each other or creating the impression that you’re disrespecting the flag. We will be non-supportive of each other,” Jones said. “We will not disrespect the flag.”Trump’s comments stoked a political controversy that had subsided somewhat since Kaepernick started it a year ago. The NFL defended the rights of players to protest.Lead NFL spokesman Joe Lockhart has said, “Everyone should know, including the president, this is what real locker room talk is.” It was an apparent reference to the “Access Hollywood” tapes in which Trump bragged about sexually assaulting women.
